Please note that we are not in-network with any insurance providers; however, our team will gladly assist you with filing claims through your insurance carrier.

Insurance Information
As a courtesy, we will submit claims to your insurance provider and assist in tracking claim status. Please notify our office of any changes to your insurance coverage prior to treatment.
You are responsible for all fees charged by our office regardless of insurance coverage. Most insurance companies respond to claims within four to six weeks. Any remaining balance after insurance processing is the patient’s responsibility.
